Collaboration between hospital and primary care can improve the management of diabetic nephropathy

The incidence of end-stage renal failure (ESRF) due to diabetic nephropathy is increasing. Nicola Thomas considers the role of patient empowerment as a tool to reduce the onset and progression of this condition. She outlines a study to facilitate an understanding of the scope for improving renal management of diabetes in primary and secondary care.
